| 3 Years | Physical with Urinalysis |
| 4 Years | Urinalysis & Vision Screen, Varivax, MMR |
| 5 Years | Urinalysis & Audiogram, Kinrix(DTaP/Polio combo), PPD (TB Test) |
| 6 Years | Urinalysis, Vision Screen, CBC & Lipid Panel |
| 8 and 10 Years | Urinalysis & Vision Screen |
| 11 or 12 Years | Urinalysis & Vision Screen, Tdap, Menactra, HPV vaccine (females) |
| 14 Years thru 18 Years | Physical with UA & Vision, Chlamydia screen girls 16 years & older or sexually active |
